On the dueling front, impromptu duels could be fun, but would require everyone to likely to become embroiled in them to submit 12 dueling actions to rinse and repeat (perhaps multiple ones for use against different weapons) together with (accept) surrender / refusal conditions. Any mandatory rests could be inserted by the GM as late as possible in the 12 actions.
The problem with D20 systems in the above circumstances is that those characters who start off good at dueling (beefy and/or with high expertise) tend to get increasingly better (thanks to dueling victories) while those who aren't tend to fall farther and farther behind. I ran one of the former in a game years ago - fun for me, but not so much fun for those whose characters he crossed swords with...

I think TPT may have been a face to face game, not sure on that. (It did center round an IRL gaming cub).

With regards to players opting out of sections of the rules, frankly I disagree mightily. We either play on a level playing field or we don't is my opinion. If everyone has the option of either being subject to individual random events or opting out how is that not a level playing field...? I have no problem with players being able to choose to become uproariously drunk - or deciding not to carouse at all. While I understand your concern here, the capacity of being surprised by a random event is increased if you don't know which may they be,so I keep thinking it would be better if the players do not know the list of possible ones. Of course, as our king Louis XIII said, we can all contribute to them by suggesting him some, but he will be the ultimate arbiter about which ones may enter and how will they be applied.

I just don't see the appeal or necessity for them. I was under the impression this is a role playing game. Not a random number generator output. IMO we already have little to no player interactions from month to month, random events aren't going to increase that. But I've made my position clear. I'm not in favour. Others are. Majority (and KLXIII) rule.
